Two Ways We Work

Provider Care — Right at Home.

There will be two ways to work with us. Families and patients will arrange in-home and telehealth provider visits directly. Home health agencies will partner with us for patient evaluation, establishment of care, and ongoing medical oversight.

Part One — For Families & Patients

Provider visits that come to you

Some patients have difficulty getting to a traditional medical office, but they still deserve timely, attentive, high-quality medical care. Families and patients will be able to arrange in-home and telehealth visits with our provider directly, with no agency or referral needed.

Services May Include

  • Comprehensive new-patient evaluations
  • In-home and telehealth provider visits
  • Post-hospital and transitional care follow-up
  • Chronic disease management
  • Medication management and reconciliation
  • Evaluation of changes in condition
  • Wound and skin evaluations
  • Review of laboratory and diagnostic results
  • Coordination with specialists
  • Ongoing provider follow-up and medical oversight

If you're already receiving skilled home health services and simply need an established provider to evaluate you and provide ongoing medical oversight, this is the path for you. Sign up below to be notified when home visits open.

Part Two — For Home Health Agencies

A provider partner for your home health patients

This is a separate track from the in-home provider visits above. Home health agencies will be able to refer patients to us for evaluation and establishment of care. Once a patient is established, our provider will offer ongoing medical oversight, collaborate with the agency, and review and sign clinically appropriate orders based on the patient's individual needs.

We won't replace your agency's home health services. We'll serve as the patient's medical provider and collaborate closely with your clinical team.

Who we can help with

If you have a patient who needs to establish care with a provider, requires a face-to-face evaluation, needs follow-up after hospitalization, or needs an eligible provider to sign and manage medically necessary home health orders, we'd like to hear from your agency as we prepare to launch.

Our goal

To become a responsive, established provider your agency can count on — someone who evaluates the patient, provides ongoing medical oversight, and signs appropriate orders so your team can focus on delivering the skilled services.

How Referrals Work

A simple, four-step process

Here is how referrals will work.

01

Refer Your Patient

The home health agency contacts The Health Standard regarding a patient who needs provider support.

02

We Establish Care

Our provider evaluates the patient and determines whether our practice is appropriate for their ongoing medical needs.

03

We Coordinate With Your Agency

Once established, our provider communicates with the home health agency regarding medically necessary orders and changes in condition.

04

We Continue Provider Oversight

Our practice provides ongoing medical management and reviews clinically appropriate orders based on the patient's needs.

Service Area

Where we see patients

In-home visits

The in-home service area will be announced before launch.

Telehealth visits

The state(s) where telehealth visits are available will be announced before launch.

Part Two, Continued

A provider your agency can communicate with

Once a patient is established with us, our provider plans to sign and manage clinically appropriate:

  • Skilled nursing orders
  • Physical therapy orders
  • Occupational therapy orders
  • Speech therapy orders
  • Home health aide services
  • Wound care orders
  • Laboratory orders
  • Durable medical equipment needs
  • Medication-related concerns
  • Changes in patient condition
  • Plan-of-care documentation
  • Recertification and continued-care needs

All orders and documentation are reviewed based upon the patient's individual clinical condition, applicable regulations, payer requirements, and the provider's independent medical judgment.
