Who we are
The Health Standard is a nurse-founded practice in development. We are not yet seeing patients. This website shares our plans and lets you sign up for updates. This policy explains what the website collects and how we use it.
What we collect
Information you give us through our forms. Depending on the form, this may include:
- Your name and email address
- Your phone number and ZIP code, where a form asks for them. On our launch-update forms these are optional.
- The choices you select, such as the service you are interested in, whether you are a patient or a family member or caregiver, or your visit preference
- For home health agencies: the agency name, a contact name, email, phone number, and the reason for reaching out
Basic technical information. Our website host, Netlify, may automatically record standard technical details such as IP address, browser type, and the time of a visit in order to deliver and protect the site.
This website does not use advertising or analytics trackers.
Please do not send health information. This website, its forms, and our email address are not secure channels for health information. Do not send symptoms, diagnoses, medications, test results, or medical forms. If health information is sent to us by mistake, we may delete it.
How we use it
- To send the updates you signed up for, such as practice-development news, service announcements, and notice when appointments or services open
- To respond to inquiries, including home health agency partnership inquiries
- To understand which services people are interested in, and where, so we can plan
- To keep the website working and to prevent spam
Who we share it with
We do not sell your personal information. We share it only:
- With service providers that help us run this website and send updates, such as Netlify, which hosts the site and processes form submissions, and MailerLite, which sends our emails (we share only your name and email address with MailerLite)
- When the law requires it
How long we keep it
We keep your information until you ask us to remove it or we no longer need it for the purposes described above.
Your choices
You can ask us to remove you from our lists, or to correct or delete the information you gave us, by emailing Kimberly@thehealthstandard.co.
Joining a list does not make you a patient
Signing up for updates does not create a patient-provider relationship. If the practice opens, patient health information will be handled separately from this website, and patients will receive a separate notice explaining how it is protected.
Links to other websites
This website links to other sites, including MindBridge (findmindbridge.com). Those sites are separate from this one and have their own privacy practices.
Children
This website is intended for adults. We do not knowingly collect information from children under 13.
Security
We take reasonable steps to protect the information you give us. No website or email is completely secure, which is one reason we ask you not to send health information.
Changes to this policy
If we change this policy, we will post the new version here and update the date at the top.
