FMLA Paperwork
When you or someone you love needs time away from work for a serious health condition, the paperwork shouldn't add to the stress. Our provider plans to complete FMLA medical certification forms for established patients, based on a real evaluation, returned promptly, and done right.
What is FMLA?
The Family and Medical Leave Act (FMLA) is a federal law that allows eligible employees of covered employers to take unpaid, job-protected leave for certain medical and family reasons. Your employer may ask for a certification from a health care provider to support your request — that's the part we'll help with.
Your own serious health condition
Certification for leave when a serious health condition makes you unable to perform the functions of your job — often using the Department of Labor's form WH-380-E or your employer's own form.
Caring for a family member
Certification for leave to care for a spouse, child, or parent with a serious health condition — often using form WH-380-F. This includes families caring for a loved one at home.
Continuous or intermittent leave
Some conditions call for a block of time off; others call for leave in separate pieces or a reduced schedule, such as for flare-ups or recurring appointments. The form documents which applies and why.
Recertifications and updates
Employers may request recertification over time or when circumstances change. Established patients will be able to request updated paperwork as their care continues.
Evaluation first, paperwork second
FMLA certification is a medical document, so it always starts with a provider visit — virtual, or in your home through our In-Home Provider Care service.
Schedule a visit
Book an evaluation for yourself, or for the family member you're caring for. Have your employer's form and deadline handy.
Provider evaluation
Your provider reviews the condition, your history, and your records, then completes the certification based on their own clinical assessment.
Secure return
The completed form is returned to you through a secure channel so you can submit it to your employer or HR department.
Please note: Your employer — not The Health Standard — decides whether you are eligible for FMLA and whether leave is approved. Our provider can only certify what is medically supported by their own evaluation, and cannot guarantee approval, backdate forms, or complete paperwork without a visit.
Employers generally must give you at least 15 calendar days to return a medical certification. Reach out as early as you can so there's time for a proper evaluation.
Any fees will be clearly disclosed before launch.
